At Trinity Aesthetic Studio (“Trinity,” “we,” “our,” or “us”), we respect your privacy and provide residents of California—and all users where legally required—the ability to submit privacy-related requests. This Data Subject Access Request (DSAR) page explains your rights and how to exercise them.

1. Your Rights

Depending on your residency and applicable law (including the California Consumer Privacy Act (CCPA) as amended by the CPRA), you may have the following rights regarding your personal information:

1.1 Right to Know / Access

You may request that we disclose:

  • The categories of personal information we have collected about you;

  • The specific pieces of personal information we hold;

  • The categories of sources from which the information was collected;

  • The business or commercial purposes for collecting or disclosing your information;

  • The categories of third parties to whom we have disclosed personal information.

1.2 Right to Delete

You may request that we delete personal information we have collected from you, subject to certain exceptions (for example, legal or medical record retention requirements).

1.3 Right to Correct

You may request that we correct inaccurate personal information we maintain about you.

1.4 Right to Opt-Out of Sale or Sharing

Trinity Aesthetic Studio does not sell or share personal information for cross-context behavioral advertising.
If this ever changes, we will update our Privacy Policy and provide a clear opt-out mechanism.

1.5 Right to Limit the Use of Sensitive Personal Information

If we collect sensitive information (e.g., medical information you voluntarily provide), you may request that we limit its use to what is reasonably necessary to provide services.

1.6 Right to Non-Discrimination

We will not deny services, charge different prices, or provide a different level of service because you exercised your privacy rights.


2. How to Submit a DSAR Request

You may submit a request using any of the following methods:

You may call and request instructions for submitting a written DSAR.

We will confirm receipt of your request within the legally required timeframe.


3. Verification of Identity

To protect your information, we must verify your identity before fulfilling any DSAR request.
Verification may include:

  • Providing specific pieces of personal information we already have on file;

  • Confirming your contact information;

  • Providing documentation or proof of residency (if required by law).

If we cannot verify your identity, we may be unable to fulfill your request.


4. Authorized Agents

You may designate an authorized agent to submit a request on your behalf.
We may require:

  • Written and signed permission from you;

  • Proof of the agent’s identity;

  • Direct verification from you confirming the agent is authorized.


5. Timing & Response Process

We typically respond within 45 days of receiving your request.
If additional time is needed (up to an additional 45 days), we will notify you.

We may deny a request if:

  • We cannot verify your identity;

  • An exception applies;

  • The request is excessive, repetitive, or manifestly unfounded.

If denied, we will explain why.


6. Information We Cannot Provide

There are certain categories of data we legally cannot disclose, such as:

  • Security-related information;

  • Internal system logs tied to fraud prevention;

  • Information that would compromise the privacy or safety of others;

  • HIPAA-protected health information provided strictly in the context of medical treatment (covered by a separate Notice of Privacy Practices).


7. SMS / Text Messaging (TCR Compliance)

To comply with The Campaign Registry (TCR) and carrier policies, DSAR requests may also apply to SMS data.

You may request:

  • What SMS-related data we have collected (e.g., phone number, time stamps, consent logs);

  • Correction or deletion of your SMS consent record (unless legally required to retain);

  • To stop receiving SMS messages by replying STOP to any message.

We do not sell or share your mobile number, SMS consent data, or SMS message content with third parties for their marketing purposes.

Dr. Oliak is a board-certified surgeon who spent the first 20 years of his career as a bariatric surgeon. He was one of the first surgeons to offer laparoscopic weight loss surgery in Orange County, and has helped more than 3000 patients to successfully lose weight and keep it off. 

 

In 2023, Dr. Oliak started MD Total Wellness to offer what our healthcare system doesn’t – natural, root-cause treatments, like hormone therapy, GLP-1 therapy, and peptide therapy, that improve quality-of-life, yet are overlooked by our healthcare system.  

 

In 2025, Dr. Oliak and Dr. Krishnan started Trinity Aesthetic Studio to offer transformational aesthetic solutions based on science. The innovations in energy-based aesthetics over the last 20 years has been remarkable.
